Leveraging our 2024 Accomplishments to Advance Standards in 2025: Highlights from the Gravity Project Year-End Update

Thanks to the dedicated efforts of Gravity Project’s community members, volunteers, and staff, we made significant progress in advancing standards for Social Determinants of Health (SDOH) in 2024 that will shape the future of health data sharing.

Key Achievements

Gravity focuses on developing consensus-based standards that enhance data interoperability for SDOH. Our terminology, technical, and implementation workstreams all made notable progress in 2024 that we can leverage this year.  We provided valuable resources and guidance to help implementers adopt Gravity terminology effectively.

One of our standout initiatives was a six-week discovery project in February 2024, where over 20 cross-sector stakeholders collaborated to explore social care payment and reporting standards. This effort not only resulted in a comprehensive report but also fostered a deeper understanding of the landscape surrounding social care.

Member-Led Initiatives

The commitment of our members was evident in various initiatives aimed at bridging the gap between health and social care data. For instance, we developed an open-source classification of social care program categories to facilitate service navigation. This alignment with the United States Core Data for Interoperability (USCDI) enhances access to community-based services for both providers and individuals.

Advancements in Terminology and Technology

Our Terminology Workstream made significant headway in 2024, expanding our consensus on social risk terminology and addressing new domains such as Utility Insecurity and Language Access. These efforts ensure that our terminology remains relevant and impactful across national quality measures.

Additionally, the Technical Workstream released an updated Social Determinants of Health Data Exchange Implementation Guide, incorporating real-world feedback to streamline future implementations. Our ongoing collaboration with Health Level Seven® International (HL7) ensures that our standards align with other crucial health IT frameworks.

Moving Forward

As we continue moving forward in 2025, our focus remains on expanding and diversifying our membership to foster collaborative standards development. We are committed to creating sustainable standards that address the real-world needs of diverse stakeholders and communities.
